You must complete the following steps PRIOR to being permitted in any Head Start site to do observations or student practicum.

1.  Application for Criminal History Registry:  Complete the enrollment form, enclose a check or money order for $3.00 and mail it to:  Child Care Division, P O Box 14050, Salem, OR  98309-4050

2.  Once you receive confirmation from Child Care Division that you are enrolled in the Criminal History Registry, complete the application and Health Self Appraisal forms and fax them to Head Start Human Resources at 541-245-9188.

3. After your paperwork has been received by Head Start Human Resources and has been approved, you will be notified that you have clearance to be in the Head Start center and you will be issued a sticker for your student ID.   Be sure you have this identification with you, as you will not be permitted in the center without it.
