If you are currently a recipients of a TANF funded program (ERDC, cash grant, assessment program) or any family member living in the household is on SSI (Supplemental Security Income). ERDC Application. For application instruction, www.dhs.state.or.us/children/. This website has many options. To access instructions click "Child Care", then click "Subsidy Rates", then look for "ERDC Application". Children with disabilities need to apply.

Instructions:

2. Print the Application and Cover Letter.
3. Make copies of Date of Birth verification (Birth Certificate, Medical Card, Hospital Record, or Immunizations) and All income for the last 12 months. We must be able to verify a full year of income (W-2's for the previous year, Tax Return for previous year, Supplemental Security Income award letter, Foster Child placement letter, Paycheck Stubs for the most current 12 months, and Child Support Payments). If you are not able to gather any or all of this documentation; please call us at 734-5150 and ask to speak with Enrollment - we can help!!!
